Type 2 Diabetes

In diabetes the body is unable to break down glucose into energy, either there isn’t enough insulin to move the glucose into cells, or the insulin produced doesn’t work properly.

High blood sugar levels can seriously damage different parts of the body, including your feet, eyes and heart. Diabetes type 2 is reversible when you learn how to normalise blood sugar levels. Annual Reviews of Long Term Conditions with our Practice Nursing Team or Clinical Pharmacist

  • If you have one or more of the Long Term Conditions listed below you will be invited to book a single annual review with a Practice Nurse or our Clinical Pharmacist near your birthday.
  • For some kinds of Annual Review you will be asked to make an appointment with a Health Care Assistant or Phlebotomist before attending an appointment with a Practice Nurse or Clinical Pharmacist.

Please book your review by calling our dedicated annual review booking line  01273 224865 which is open 8:30 and 5:30 Monday to Friday.

 Annual Reviews cannot be booked Online or via our Automated Telephone system.  This is because different reviews have different requirements for staff qualifications and length of appointment, and have different pre-appointment instructions.

Asthma Annual Review

All patients with asthma are invited for an annual review. This includes all patients with a history of asthma who have required inhalers in the past 12 months.

The nurses will check that your medication is appropriate and that your asthma is well controlled.

Cardiac Annual Review

All patients with a history of heart/vascular disease, including heart failure, atrial fibrillation, strokes and peripheral vascular disease are invited for an annual review with one of our nurses.

You may be asked to see a Health Care Assistant for a blood test beforehand. The Practice Nurse will ensure that your medication is up to date.

COPD Annual Review

Patients with chronic obstructive pulmonary disease are invited for an annual review with one of our nurses.

Medication is reviewed and symptoms are assessed. Regular spirometry testing is offered to patients with COPD.   

Please bring your inhalers with you.

Diabetic Annual Review

We invite all of our patients with diabetes for an annual review unless they are being treated regularly at the hospital.

You will receive a letter each year, when your check is due, giving you information about making an appointment.  You will be asked to see a Health Care Assistant for a blood test beforehand.

Learning Disabilities Annual Review

All patients with learning disability are offered an annual review with a nurse.

The aim of the review is to address medical problems and health promotion and to manage the physical, emotional and social needs of these patients.

Mental Wellbeing Annual Review

Support for patients who have experienced some more severe mental health symptoms.

It is known that some people with more significant mental health problems have an increased risk of other health problems such as diabetes, heart disease or strokes. These clinics are here to help with lifestyle advice

Dementia Review

Support for patients with dementia and their carers, to ensure that you are up to date with health checks and in touch with other agencies to help support your care
