Repeat Prescription requests

Patients on continuing courses of treatment (called repeat medications) are issued with computerised repeat prescription order forms. When ordering further courses of treatment please tick the items you require on your repeat order form.

Ordering your prescription:

• Post your repeat order form in the prescription box in our entrance lobby (blank forms are available) OR
   
• Fax your request to us on 220883 OR
   
• Order online. (See the EMIS Access link below.) OR
   
• Many local pharmacists deliver repeat order forms every day

Please note that we are unable to take repeat requests over the telephone.

Please allow us two full working days to process your request

WHAT HAPPENS TO YOUR PRESCRIPTION – YOUR CHOICE

What happens to your prescriptions is your choice. If you want to change what usually happens to your prescriptions, please
- tell reception OR
- send us a message using your EMIS Access account

Pharmacy

You can choose any local pharmacy to collect your prescription from reception. Please note: If no pharmacy is specified prescriptions will not be passed to a pharmacy for dispensing. If you sign a form at your pharmacy we will need to see this the first time that the pharmacy collects your prescription. Click on the nhs choices website www.nhs.uk to find a list of local pharmacies by postcode.

COLLECT

You can choose to collect your prescription yourself from reception and hand it to a pharmacy of your choice.

POST

You can choose to give us a Stamped Self Addressed Envelope with your request so that we can post your prescription to you.

ON-SITE PHARMACY

We regret to announce that our on-site pharmacy has closed. We hope to have on-site pharmacy again soon.

PRESCRIPTIONS PREVIOUSLY COLLECTED FOR HOME DELIVERY BY THE ON-SITE PHARMACY (ACKERS) will be collected and delivered by their other branch:
