Charter Patient Reference Group (PRG) – a ‘virtual’ group |
We want to ask a wide range of patients for their views.

We would prefer to
contact you by email or text. You will become part of our ‘virtual’ Patient
Reference Group (PRG). Your opinions will be anonymous and your contact details
will be kept safely*. There is no time commitment or meetings involved!
Members will usually be contacted by email or text and asked to complete short
anonymous surveys on a web site.
Of course, if you are unable to use email or text we will be happy to post a
survey to you.
Your contact details will not be shared with other group members and your email
address will not be stored for other purposes within the practice.
Only anonymous results will be discussed.

Charter Patient Participation Group (PPG) |

If you would like to get more involved, our PPG are a group of volunteer
patients who meet regularly. They aim to help make sure that the surgery
provides the services its patients need. For instance, they will be looking at
the results of the anonymous surveys from the PRG.
As a PPG member you will share your contact details with other group members*,
however, your email address will not be stored for other purposes within the
