We will try to register you as quickly as possible - please allow at least two full working days.

1. please check that you live within our practice area.
    
2. please check that you are eligible to join a GP list permanently - you must normally be
   • living in the UK legally and for a settled purpose
   • living in the UK for the majority of the time (more than 6 consecutive months in any year, 9 months in total)
   You may be asked to show a valid non-visitor Visa as proof of this (or Home Office documents)
    
3. complete and sign the following downloadable documents:
   • an NHS GP Registration Form – (click here for a printable version)
   • a Health Questionnaire (click here for a printable version).
   
    
4. please record your Blood Pressure, Weight and Height using the machines in our waiting room
    
5. bring the following original documents to reception with your completed forms:
   • Proof of identity: Passport or EU ID card, Photographic UK driving licence, UK birth certificate, child’s immunisation red book.
   • Proof of address (dated within the last 3 months): utility bill, tenancy agreement, bank or credit card statement, council tax bill
   
    
6. all new patients are eligible for a new patient check; if you are over 40 in particular please book a New Patient Health Check when you hand in your forms.
   
   Your appointment will be with our Health Care Assistant who has been specially trained to give Healthy Lifestyle and Disease Prevention advice and who can support you in accessing specialist services.

When registered with the practice you may book an appointment with any doctor. However, for continuity of care, you will be appointed a 'usual doctor'. Please tell us if you have a preference. Requests will always be considered but may not always be appropriate.
