Registering
With The Practice
If you live
within our
practice area and would like
to register with the practice,
please ask at reception to see if we
are currently taking on new
patients.Patients are registered with the
practice, not individual doctors.
However, in order to ensure
continuity of care, the partnership
operates a personal list system.
This means that you will be
appointed a 'usual doctor' and this
is who you will usually see.
However, if your problem is urgent
you may be offered an appointment
with another doctor.
We hope that having one doctor
responsible for your health improves
the quality of care you receive. If
you have a preference for a
particular doctor, you may like to
express this in writing on
registering. Alternatively, you may
request a specific doctor for
certain aspects of your care.
Requests of this nature will always
be considered but may not always be
appropriate.
You will be asked
to complete a registration form and
a new patient health questionnaire.
We will endeavour to register you as
soon as possible. It is helpful if
you do not delay your registration
until you need to see a doctor
urgently, as it usually takes two
working days to register a patient.
On registering, you
will need to show us two forms of
identification. One can be a
passport, driving licence, birth
certificate etc. The second must be
proof of your current address. This
could be a utility bill, rent book
or bank statement. Please ask at
reception for a full list of
acceptable identification.
