If you live within our practice area please ask at reception if we are currently taking on new patients. If so we will register you as soon as possible – please allow at least two full working days. Please do not delay registration until you need to see a doctor urgently.

1. You will be asked to complete a Registration Form. Before you can register you will need to show us:

• Proof of Identity: Passport or EU ID card, Photographic UK driving licence, UK birth certificate.
   
• Proof of address (dated within the last 3 months): utility bill, rent book, bank statement, council tax bill

2. You will be asked to complete a Health Questionnaire. If you can please:

• Use our Blood Pressure Machine
   
• Use our Weight / Height machine to check your Body Mass Index

3. We would like you to book a New Patient Health Check

• We will shortly be asking new patients to book a New Patient Health Check as soon as possible after registration. Your appointment will be with our Health Care Assistant who has been specially trained to give Healthy Lifestyle and Disease Prevention advice and who can support you in accessing the specialist services that are on offer.

4. You will be registered with the practice, not an individual doctor.

• However, for continuity of care, you will be appointed a 'usual doctor'. You may request a different doctor for certain aspects of your care and if your problem is urgent you may be offered an appointment with a different doctor. If you want a particular doctor as your ‘usual doctor’ please tell us. Requests will always be considered but may not always be appropriate.
